Schedule Your Appointment
We can’t wait to meet you and your pet! To get started, all new client appointments must be scheduled by phone. This allows our team to learn a little about your pet, match you with the right appointment type, and answer any questions you may have.

New Client Form
Save time at check-in by filling out our New Client Form ahead of your first visit. This gives us important details about you and your pet so we can focus on providing the best care from the moment you arrive.
What to Bring to Your First Visit
To make your first appointment smooth and stress-free, please bring:
- Any previous veterinary records (or email them to us at info@heartlandveterinaryil.com).
- A list of your pet’s current medications, supplements, flea/tick, or heartworm preventatives.
- The brand and type of food and treats your pet eats.
- Any recent urine or stool samples if requested by our team.
- A list of contact phone numbers for you (and anyone else responsible for your pet’s care).
Please note: Anyone bringing a pet to their appointment must be at least 18 years old and able to make medical and financial decisions.
Payment Options
We strive to make your pet’s care as accessible as possible by offering flexible payment methods. For your convenience, we accept:
- CareCredit
- All major credit cards (with a 3% surcharge applied to all credit card transactions. Debit cards do not have a surcharge)
- Cash
- Checks
Please note: A deposit may be required for surgical or anesthetic procedures.

Tips for a Great Visit
We know vet visits can sometimes be stressful for both pets and people! Here are a few ways to make the experience easier:
- Safety first: Please keep dogs on a leash and cats/small pets in a secure carrier.
- Arrive early: A few extra minutes allows time for paperwork and helps your pet settle in.
- Share everything: Don’t worry about being judged. We just want the full picture so we can give the best care possible.
- Ask questions: If you’re ever unsure about what we’re doing or why, just ask. We’re here to keep you informed and comfortable every step of the way.
